A Notice to Our Patients of a Cyber Incident

Woodholme Gastroenterology Associates, a LifeBridge Health Partner (“Woodholme”) takes the privacy and confidentiality of our patients’ information very seriously. Regrettably, this notice is to advise our patients of an incident that may have involved some of that information.

On March 1, 2021, we learned that an unauthorized third party gained access to our network and obtained certain files from Woodholme’s systems on February 25, 2021, some of which may have contained limited patient information. Following a comprehensive review of all potentially impacted computer files, we determined that some patients’ names, addresses, email addresses, dates of birth, patient identification numbers, diagnoses and/or treatment information may have been contained in the files. In limited instances, the files also included patients’ Social Security numbers, driver’s license numbers, and/or health insurance information.

We have no indication any patient information has been misused, however, we wanted to advise our patients of the incident and assure them we are taking this matter seriously. For those patients whose Social Security numbers and/or driver’s license numbers were involved, we are offering complimentary credit monitoring and identity protection services.

We have established a dedicated call center to answer questions patients may have about this incident, which may be contacted at 1-888-665-6553, Monday through Friday, between 8:00 a.m. and 5:00 p.m. Eastern Time. We also recommend our patients review the statements they receive from their healthcare providers. If the patients see charges for services they did not receive, they should contact the provider immediately.

We deeply regret any concern or inconvenience this may cause our patients. Woodholme takes the privacy and security of patient information very seriously and had security measures in place to protect against cyber risks. As the cyber threat landscape continues to evolve, we are continuing to take steps to enhance the security of our systems and the information we maintain.
